Abstract:

Several Sustainable Development Goals (SDGs) (3, 16, 17) point to the need to systematically address massive shortages of human RESOURCES for health (HRH), build capacity and leverage partnerships to reduce the burden of global illness. Addressing these complex needs remain challenging, as simple increases in absolute numbers of healthcare providers trained is insufficient, substantial investment into long-term high-quality training programs is needed, as are incentives to retain qualified professionals within local systems of care delivery. We describe a novel HRH initiative, the Global Health Service Partnership (GHSP), involving collaboration between the US government (President’, s Emergency Plan for AIDS Relief [PEPFAR], Peace Corps), 5 African countries, and a US-based non-profit, Seed Global Health. GHSP was formed to enlist US health professionals to assist in strengthening teaching and training capacity and focused on pre-and in-service medical and nursing education in Malawi, Tanzania, Uganda, Eswatini and Liberia. From 2013-2018, GHSP sent 186 US health professionals to 27 institutions in 5 countries, helping to train 16 280 unique trainees of all levels. Qualitative impacts included cultivating a supportive classroom learning environment, providing a pedagogical bridge to clinical service, and fostering a supportive clinical learning and practice environment through role modeling, mentorship and personalized learning at the bedside. GHSP represented a novel, multilateral, public-private collaboration to help address HRH needs in Africa. It offers a plausible, structured template for engagement and partnership in the field.

Abstract:

Background: Emerging evidence that meaningful relationships with knowledge users are a key predictor of research use has led to promotion of partnership approaches to health research. However, little is known about health system experiences of collaborations with university-based researchers, particularly with research partnerships in the area of health system design and health service organization. The purpose of the study was to explore the experience and perspectives of senior health managers in health service organizations, with health organization-university research partnerships. Methods: In-depth, semi-structured interviews (n = 25) were conducted with senior health personnel across Canada to explore their perspectives on health system research; experiences with health organization-university research partnerships; challenges to partnership research; and suggested actions for improving engagement with knowledge users and promoting research utilization. Participants, recruited from organizations with regional responsibilities, were responsible for system-wide planning and support functions. Results: Research is often experienced as unhelpful or irrelevant to decision-making by many within the system. Research, quality improvement (QI) and evaluation are often viewed as separate activities and coordinated by different responsibility areas. Perspectives of senior managers on barriers to partnership differed from those identified in the literature: organizational stress and restructuring, and limitations in readiness of researchers to work in the fast-paced healthcare environment, were identified as major barriers. Although the need for strong executive leadership was emphasized, “ multi-system action” is needed for effective partnerships. Conclusion: Common approaches to research and knowledge translation are often not appropriate for addressing issues of health service design and health services organization. Nor is the research community providing expertise to many important activities that the healthcare system is taking to improve health services. A radical rethinking of how we prepare health service researchers; position research within the health system; and fund research activities and infrastructure is needed if the potential benefits of research are to be achieved. Lack of response to health system needs may contribute to research and ‘ evidence-informed’ practice being further marginalized from healthcare operations. Interventions to address barriers must respond to the perspectives and experience of health leadership.

Abstract:

Construction PARTNERING between the landowner and the builder for the construction of condominium contract is one of the most common and widely used contracts. In the meantime, one of the challenging and controversial issues is the repudiation (termination or cancellation) of construction PARTNERING before the construction of pre-sale condominium and its impact on the pre-buyers of the condominium share. The view that considers the nature of the pre-sale contract in the contract of construction PARTNERING as its common and traditional sense, with the effect of the issue of possessory of the contract of sale (Arabic: بَیْع, Bayʿ) and principles such as the principle of correctness (validity) and the principle of non-authorization of the impact of termination (repudiation) and at the same time, regardless of the nature and concomitants of the contract of construction PARTNERING, believes that the subsequent termination does not affect the previous pre-sales. In contrast, according to the second point of view that considers the pre-sale of the builder’s share as a contractual and continuous covenant in the construction of condominium contract has different and specific elements than the common and term concept of sale (Arabic: بَیْع, Bayʿ) even with the knowing of ownership of all types of contracts of sale and considering the principle of mutual consent (agreement) and the structure of the construction PARTNERING contract and complete dependence of the pre-sale contract on that contract with the termination of the PARTNERING contract before its enforcement had been cancelled the possibility of delivery of the object of sale in the context of PARTNERING contract as the mother and the basis of pre-sale contracts and pre-sale contracts are subjected into forcible automatic cancellation. This article has proved the second theory by explaining and analyzing the nature of the contract of construction PARTNERING and its relationship with pre-sale contracts in the form of maternal-fetal attachment (MFA) and conditional contract and considering the principle of voluntarism (dominion of will or supremacy of will), and considers as a remover of some legal challenges of pre-sale apartment contracts resulting from construction PARTNERING contract in Iran’s judicial precedent.

Abstract:

Harirud river with an international nature is shared between Afghanistan, Iran and Turkmenistan and currently there is no treaty among these states regarding the division of its water RESOURCES. In recent years, after the construction of the Doosti Dam by Iran and Turkmenistan, Afghanistan, as an upstream state is moving towards hydro-hegemonic policy in the region by building various dams which restrain and divert the water flow of this river in order to decrease the downstream countries’ water rights. On the other hand, due to the high importance of Harirud water RESOURCES for Iran, continuing this type of behavior by Afghanistan might cause conflicts and create security risks among these countries in the near future. Therefore, employing a descriptive-analytical approach, the study aims at examining the issue according to international legal documents in the field of sharing common water RESOURCES. Finally, the findings indicate that the best and most practical method for exploitation of Harirud water RESOURCES is based on the theory of limited territorial sovereignty and implementation of the principle of equitable and reasonable utilization of common water RESOURCES by Afghanistan, without causing significant harm to downstream states.

Abstract:

It is widely accepted that research evidence should inform policy and practice in health service organizations. Yet, amid increasingly complex and even wicked realities, where health inequities prevail and resource-strained health service organizations struggle to keep pace with demand, using research to inform practice and policy remains an elusive ideal. Bowen and colleagues’ study illuminates critical relational pathways for engagement in evidence-informed practice and decision-making and suggests beginning insights into what might contribute to the tenuousness of this aspirational ideal. But what kind of reimagination is needed to move toward more genuine engagement in research? This commentary argues for reimagining the relationship between researchers and health research, positioning researchers as responsive, guided by humility, and part of a greater collective effort to advance a public good. It challenges notions of objectivity and detached expertise, suggesting that researchers embrace an active practice of humility focused on approaching research in service and from a position of learning rather than knowing.
